Log-in Instructions  How to log into the aforementioned site. You will need your EMPLOYER ID which you can obtain from your HR Department or by contacting us directly by phone at 800-600-0947 or email 125info@murfeemeadows.com. You will always use the EMPLOYER ID in place of your Card Number when registering your account. The EMPLOYEE ID will be your SSN with no spaces or dashes.

 

Online Claim Submission Instructions: You will first have to register an online account to access the claims portal.  Once you set up online access, you can submit claims online via http://mmi.wealthcareportal.com.

 

COBRA  Account Information: To confirm your contact information, paid through date and premium amount due, please contact us directly at COBRA@murfeemeadows.com for account information.
